To become a medical cannabis patient, you must be legally residing in Washington State and have a qualifying medical condition. Typical conditions include chronic or intractable pain, muscle spasticity, diseases which cause nausea, vomiting, wasting, appetite loss, cramping and seizures.

Getting to our Clinic
We are off exit 148th Ave NE (South Exit) on I-520. Take a left on NE 24th St (the first cross street off the exit). Go two short blocks and take another left on 152nd Ave NE. As you head north you will see the Overlake Business Center on your left. Look for our address in building #6, our practice is located conveniently facing the main street and features an abundance of accommodating free parking in the business lot.

Chart Notes (two to five most recent visits) and Labs
Authorization Payment and Co-pay (for your convenience we accept Visa & MasterCard)
No access to medical records? Patients may have records released to our doctor by completing a medical release form in the clinic.
End of Appointment
All patients receive their medical authorization the same day as their appointment. Each patient will receive three original signed copies of the legal recommendation printed on tamper-resistant paper. Patients are added to our proprietary patient database for verification purposes at the conclusion of each appointment. Patients may begin a new journey toward alternative healing.
